Social Services and Case Management
Our Social services and Case Management team assists both residents and their families during transitional periods. They also provide essential information, manage requests and concerns, help in care planning and manage discharge planning for each resident.
Social services can arrange professional services for:
- Dementia assessment
- Individual therapy
- Arrange transportation to appointments
- Accompanying resident to appointments upon request
- Power of Attorney
Case Management can arrange special services for:
- Psychosocial and Psychiatric Evaluation
- Podiatry
- Optometry
- Audiology
- Dentistry
Personal Companion Program
At Laurel Ridge Rehabilitation & Skilled Care Center we offer a personal companion program. We understand that an admission to an extended care setting can be a new and unfamiliar process for many, and it’s reassuring to have support by your loved one’s side.
With our Personal Companion Program, transitioning from the hospital just got easier. Upon admission to our center, your loved one will be assigned their own temporary personal companion to provide the short-term, personalized attention they need to get adjusted to their new setting. Whether it’s just a little TLC to settle in, or additional supervision for behavior, medical, or safety reasons, this short-term program helps provide a positive transition. You can feel comfortable knowing your loved one will have an experienced professional by their side for anything they need.
